Register a birth and get a B-form

پیدائش سرٹیفکیٹ / ب فارم
Births are registered at the local union council, and NADRA then issues the child's B-form (CRC).

Last checked November 2026

Registering a birth is a legal requirement and the first step in a child's official identity. The union council (local government) records the birth. NADRA then issues the B-form (Child Registration Certificate), which is needed for school admission, passports and later the CNIC.

What to bring

  • Hospital birth slip (if available)
  • Both parents' CNICs
  • Nikah nama (if asked)
  • Union council birth certificate (for the B-form)

Take originals plus photocopies. Offices may ask for extra documents.

Your route

  1. 1

Register the birth at your union council or local government office, using the hospital birth slip if you have one.

  2. 2

Get the computerised birth certificate from the union council.

  3. 3

Visit NADRA with the birth certificate and both parents' CNICs to get the B-form (Child Registration Certificate).

  4. 4

Keep both documents safe. You'll need them for school admission and passports.

Tips & common mistakes

  • Register the birth soon after delivery, because late registration can need extra paperwork.
  • Make sure the child's name is spelled exactly the same on both documents.

Frequently asked questions

What's the difference between a birth certificate and a B-form?+

The birth certificate comes from the union council and records the birth. The B-form comes from NADRA and registers the child in the national database.
